We make every effort to see our patients at their appointed
times. It is usually not possible to see patients prior to their appointed
times. Please note that treatments and procedures are done at various
times during the day and other patients may come and go while you are
waiting because they are on different schedules.
Emergencies
Patients are seen by appointment or as soon as possible in emergencies.
Dermatologic emergencies include:
sudden onset of a severe rash with intolerable itching or pain
severe blistering
loss of skin
rapidly progressive infections
possible cutaneous anthrax
other possible bioterrorism skin problem
First
Time Appointments
Be prepared for a longer visit if this is your first visit with us. This
will allow us to make a thorough evaluation of your condition. Expect
to spend about 15 minutes on preliminary paperwork. Please
bring your insurance card and current medications.
In accordance
with the government's HIPAA privacy guidelines, all new patients
will be provided with a Notice of Information Practices and Consent
to the Use and Disclosure of Health Information for Treatment, Payment,
or Healthcare Operations.
Surgery
and Other Treatments
All surgeries and some treatments will require a consultation and examination
before scheduling in order to provide a thorough diagnosis and prescribe
an effective treatment plan. In order for us to comply with insurance
plan rules, insurance information and authorizations must be available
when you make your appointment.
Cancellations
We ask that you let us know as soon as possible if you need to cancel
an appointment so that the time slot can be made available to others.
Call early in the morning to check for cancellations if you would like
to be seen that day.
After
Hours
After hours, an answering machine is available for routine calls. After
hours emergency patients may call our answering service at (831) 754-8631
and they will contact Dr. Lofgren. If you have an immediate life-threatening
emergency, you should go immediately to an emergency room or call 911.
Insurance plan contracts require that co-payments be
made at the time of your visit. For non-contracted insurance plans, payment
is expected at the time of service, and a statement will be provided so
that you can submit an insurance claim. Please call to verify insurance
plan participation.
